MMA is the test that closes the loop on the gray-zone B12 question. When a patient has been on metformin for 10 years, their serum B12 is 310 pg/mL, and they have had neuropathy symptoms for 2 years, I want to know their MMA. If it is elevated, I now know that functional B12 deficiency has been contributing to their neuropathy. Methylcobalamin 2,000 mcg sublingual daily and recheck at 3 months. The improvement in neuropathy symptoms when this is caught is one of the most meaningful interventions I make, and the only reason it was missed for two years is that nobody checked MMA.

Serum B12 of 310 pg/mL is labeled normal. MMA of 0.58 mcmol/L says the cells disagree.
