Medical Records
Records Release and Transfer
Medical records can only be released with your written authorization. You are responsible for obtaining prior records from other physicians and healthcare providers. Please contact those providers directly and arrange for records to be sent ahead of your appointment.

Consultations & Initial Visit
Initial Consultation
Your first visit includes a 45-minute medical consultation with Dr. Lamkin. Nutritional therapy and laboratory or diagnostic testing are often integral components of your treatment plan. Lab results are used to design your personal healthcare program and to identify the root causes of your condition.
Many lab tests require an 8-hour fast. You may drink water and should take your regular medications. Avoid coffee, sugary drinks, and food prior to your lab draw. Your out-of-pocket cost for any recommended testing will be reviewed with you before labs are ordered. All final lab results are reviewed with you at your follow-up visit.
Insurance & Payment
Cash-Based Practice
The Lamkin Clinic is a private-pay clinic. We do not contract with any insurance company and do not bill insurance directly for visits and consultations. Payment in full is required at the time of service.
We accept cash, checks, Visa, MasterCard, Discover, and American Express. An active credit card on file is required to reserve your appointment.
We will provide a detailed receipt with diagnosis codes for all services. You may submit this to your insurance carrier to seek reimbursement. In most circumstances, insurance can be used for outside laboratory and diagnostic testing ordered during your consultation. You will be informed prior to testing which labs accept insurance and which are private-pay.
Appointments & Cancellations
48-Hour Cancellation Policy: Due to the significant time reserved for patient consultations, cancellations must be made at least 48 business hours prior to your scheduled appointment. Cancellations made after this window will be charged the full amount of time reserved. If calling after hours, please leave a message.
Appointment Reservation
All appointments must be reserved with a credit card or prepayment. No charges will be applied unless you miss or cancel an appointment without proper notice.
On the day of your appointment, all charges for consultations and laboratory testing will be itemized and reviewed with you before payment is collected.
Follow-Up Consultations
Follow-up phone consultations will be billed to the credit card on file unless you provide other payment instructions prior to the call. If additional lab kits are sent, applicable fees will be charged to your account.

- After-hours messages will be returned on the next business day if you leave a message requesting a callback. If you do not leave a message, we have no way of knowing you called.
- For medical emergencies, call 911 or go directly to the nearest emergency room.
When Leaving a Message
Please include your full name and date of birth, reason for the call, best time to be reached, phone number, and email address if desired.
Prescription Refills
Allow 72 hours for prescription refill processing. Please plan ahead to avoid interruptions in your medications. Your pharmacy may fax refill requests to (405) 285-4352. Always follow up with the clinic to confirm the request was received.
