It is important to read all the information carefully. All forms must be completed and mailed or faxed to our office preferably twenty-four hours prior to your appointment. This will allow us to review your history more efficiently prior to your visit.
Medical Records
Medical records can only be released with your authorization. You are responsible for obtaining previous medical records from other physicians or health care providers. Please contact your physician or other health care provider to obtain these records. Your records should be mailed to the address below or faxed to (405) 285-4352.

Consultations
Your initial visit will include an 45-minute medical consultation with Dr. Lamkin. Nutritional therapy and laboratory/diagnostic testing may also be an integral component of your treatment plan. Test results are used to design your personal health care program as well as uncover the root causes of your medical condition.
Insurance
In most circumstances, your health insurance can be used for outside laboratory and other diagnostic testing which will be based on your initial consultation with Dr. Lamkin. We use a number of lab companies some of which bill insurance while others are private-pay only. Towards the end of your consultation with Dr. Lamkin, recommendations for testing will be made. You will be informed prior to the testing as to what portion of the laboratory evaluation insurance may be used.
The Lamkin Clinic is and has always been (15+ years) a private-pay clinic. This means we do not contract with any insurance companies and therefore do not bill insurance directly for your visits and consultations but rather accept direct pay from our patients. You may elect to seek reimbursement from your insurance company for your visits. Diagnosis codes can be provided for your individual visits.
Initial Visits
Many of the laboratory tests require an 8-hour fast. You can drink water and should take your medication as usual but avoid coffee, sugary drinks and food prior to your lab draw. Your out-of-pocket cost for the recommended testing will be reviewed with you during your medical consultation before labs are drawn. You will receive all final lab results and be guided through their interpretation at your follow up visit.
Confirmation and Cancellation of Appointments
Due to the significant time reserved for patient consultations, there is a 48 business hour cancellation policy. All appointments must be reserved with credit card or prepayment. Cancellation must be done within 48 business hours prior to the scheduled appointment or you will be charged for the full amount of the time reserved. If calling after hours, please leave a message.
Payment Options
Our office accepts cash, checks or credit cards (MasterCard, Visa, Discover, American Express) for services rendered. When you schedule an initial visit, we request a credit card on file to hold the appointment for you. No charges will be applied to your credit card unless you miss or cancel an appointment without proper notice. On the day of your scheduled appointment, all charges for consultations (medical and nutritional) as well as laboratory testing will be itemized and reviewed with you. Payment is due on the day of service.
Follow-up phone consultations will be billed to your credit card on file in our office unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account.

Prescription Refill Request
It may take up to 72 hours to process a prescription refill. Please plan ahead to avoid any interruption in your medications. Prescription refills can be faxed to our office by your pharmacy.
